Independent Contractor Acknowledge Form

This project consists of the overlay and widening of Cheshire Road from a two-lane to a three-lane section between the intersections of Piatt Road and Old State Road. The work will include construction of new storm sewer and a culvert extension. The total length of work is approximately 0.9 miles.

Design Build package to replace both bridges with single span, steel beam structures with concrete decks. Also including minor approach reconstruction. See attached Project Scope documents.

The contract is to provide asphalt materials including hot-mix asphalt and Asphalt Liquids to Delaware County for it's own forces to use.

Improvement of surface drainage, installation of subsurface drainage, and the destruction of existing subsurface drain tile.

This project involves the realignment and widening of 0.51 miles along C.R. 13 (Worthington Road). Improvements and widening will also be made on 0.26 miles along C.R. 106 (Lewis Center Road). The new intersection of C.R. 13 and C.R. 106 will be developed by providing a peanut-shape single lane roundabout. Improvements will also include roundabout lighting, drainage, traffic control and drive improvements.
