SCOPE OF WORK:
• Base Bid:
o Demolition work consists of milling min. 2" of existing asphalt pavement in main parking area "A", Approx. 109,529 sq. Ft.
o New construction includes installation of minimum 2" of new asphalt and tack coat. Match existing drainage per design to storm water drains, and seal & restriping parking lot after new asphalt to match existing.
o Please refence the Section 32 17 23 “Painted Pavement Markings” on pages 29-31 within the Limited Scope Document for the pavement specifications.
• Alternate 1:
o Demolition work consists of saw cutting and removing min. 2" of existing asphalt pavement, Approx. 9,359 sq. Ft., in areas identified.
o New construction includes recompacting base and providing new pavement in areas identified.
• Alternate 2:
o New construction includes asphalt crack filling as needed, and reseal.

Scope of Work
- Demolition of the existing trap system
- Removal of existing trap, auger, and dust collection unit
- HEPA vacuum of the range floor
- Disposal of existing steel plates, dust collection unit, and auger
- Installation of a new Rubber Berm Trap, which shall be:
- Rifle rated
- Flame retardant
- Contractor removes and properly disposes of all materials consistent with al laws and industry best practices for the disposal of hazardous waste
1. Facility Access background checks required from all Contractor employees before work may begin. Contractor must submit all required forms within 3 days of contract award to allow time to process all paperwork.
2. All required permits to be obtained by contractor.
3. EDGE is required for bids exceeding $50,000. Participation should include 5% of the full bid amount including labor and materials. EDGE vendors can include labor and material suppliers.
4. Prevailing Wage rates are required for any state project that exceeds $75,000. See attached rates.
5. Contractor to provide the following compliance documentation upon contract award:
- Valid State ID – If you have not previously worked with the State of Ohio, you will need to apply for a State ID number – Please do so as soon as possible given this process can take up to (2) weeks. Please visit: https://supplier.ohio.gov/wps/portal/sp/suppliers/login
- Valid BWC certificate
- Proof of participation in the BWC Drug Free Safety Program, or signed affidavit
- Proof of Liability Insurance
