City of Lake Charles, Louisiana STORM REPAIRS – PURPLE HEART ROOF REPLACEMENT HL - 10 A. Project Summary: 1. Remove existing gravel, roof, insulation, metal deck and related sheet metal items down to the existing bar joist. 2. Install new metal decking and treated wood blocking at perimeter. 3. Mechanically attach specified Polyisocyanurate and Gypsum board to the metal deck as specified. 4. Heat fuse one ply of SBS modified base sheet over the insulation. 5. Fabricate and install all miscellaneous sheet metal items. Strip in with SBS modified base sheet. 6. Flash all wall flashing and curb type penetrations with one ply of the specified SBS modified base sheet. 7. Fabricate and install ANSI-SPRI ES-1 metal edge, new drip edge, gutters and downspouts as specified. Strip in with SBS modified base sheet. 8. Install one ply of a high-performance mineral surfaced modified cap sheet across the field of the roof and up 2” above cant strip. Immediately broadcast granules into bleed out. 9. Install one ply of a high-performance mineral surfaced modified cap sheet at all wall flashing and curb type penetrations. 10. Inspect roof area with Owner and Owner’s rep prior to surfacing. 11. After 30 days, apply two applications of the specified protective coating. 12. Issue 30 year NDL warranty with wind rider up to the design pressures in stamped wind uplift calculations.

Remove existing roof membrane, shingles and related sheet metal items down to the wood deck and gypsum deck as indicated on roof plan. 2. Disconnect and raise existing curbs as required to be 8” above finished roof surface as required. 3. At wood deck areas, repair, replace and secure decking as required. 4. At gypsum deck area, repair as specified. 5. Mechanically attach specified base sheet at all roof areas. 6. At wood deck area, loose lay specified base layer of Polyisocyanurate insulation and cover with FR slip sheet, install specified tapered insulation and gypsum board. Fasteners should penetrate into wood deck a minimum of 1. 7. At gypsum deck area, mechanically attach specified gypsum board. 8. Install specified torch applied SBS Modified roofing system. 9. Install two ply modified base flashing systems. 10. Install new drip edge, gutters, downspouts, counter flashings, leads and miscellaneous sheet metal items as specified. 11. Provide and install new pre-fabricated metal coping assembly. 12. Coat entire roof with a non-fibered aluminum coating. 13. Issue specified 30-year warranty.

CITY OF LAKE CHARLES, LOUISIANA WWTP A – BASIN A-07 SEWER REHABILITATION CAPITAL PROJECT NUMBER CP3333 I. Scope of Services: The City of Lake Charles WWTP ‘A’ is currently under a Compliance Order (CO) with LDEQ. The approved CO schedule requires the rehabilitation of 13 sub basins in the WWTP ‘A’ Basin. The Sub Basins have been prioritized and ranked in order of severity of condition through wastewater modeling of the system. Based on review of CCTV data of the Sub-Basin, recommendations for repair have been made and a cost estimate has been developed. This scope of work covers the repair of the collection system in the WWTP A Sub Basin A-07. The Work includes, but is not necessarily limited to, the following: Base Bid 1. There is approximately 10,198 LF of 8-inch through 18-inch gravity sewer pipeline to be rehabilitated as part of this project. Rehabilitation methods include point repairs, pipe bursting, cured-in-place lining, and manhole lining. This work shall also include the furnishing and installation of three new manholes. Alternate No.1 1. There are approximately 33 active service laterals to be reinstated as part of this project. 2. There is approximately 1,862 LF of 8-inch gravity sewer pipeline to be rehabilitated as part of this project. Rehabilitation methods cured-in-place lining.

Scope of Services: Based on review of the CCTV data of the large diameter cleaning and CCTV project within the University area, the assessed pipelines have been designated as critically deficient and priority one deficient. Following review of the assessment the following scope of work has been developed. Base Bid: A. There is approximately 9,315 LF of 10-inch through 12-inch gravity sewer pipeline to be rehabilitated as part of this project. Rehabilitation methods include point repairs, pipe bursting, cured-in-place lining, and manhole lining. Also included within this contract is the reinstating of approximately 45 active service laterals and cured-in-place lining of approximately 60 active services. Alternate No.1: A. There is approximately 451 vertical feet of cleaning, repairing, and lining of new manholes. Alternate No.2 A. There is approximately 2,028 LF of 8-inch through 12-inch gravity sewer pipeline to be rehabilitated as part of this project.
