City of Lake Charles, Louisiana Project No. 2019-692 Capital Project No. 3177 W. Prien Lake Road Widening (Cove Lane to Ihles Road) Clearing and Grubbing The general scope of work for Project No. 2019-692, W. Prien Lake Widening Cove Lane to Ihles Road – Clearing and Grubbing Phase, is defined as follows: • The clearing of trees and shrubs located within the existing or newly acquired servitudes and road rights of way located along W. Prien Lake Road between Cove Lane and Ihles Rd and also along Sale Rd from W. Prien Lake to Burton Lane. • The demolition of the five (5) homes located at 2804 Henderson Forest Lane, 2803 & 2804 S. Locke Point Drive, and 2801 & 2802 N. Locke Point Drive. As well as the associated removal of driveways, sidewalks, pools, sheds, fencing, etc required to completely clear the lots. All of the work identified above is being done in anticipation of the project to relocate utilities and reconstruct and widen W. Prien Lake Road and Sale Road within the same area.

PROJECT NO. 2019-690 CAPITAL PROJECT NO. 3335 KIRKMAN STREET DRAINAGE REHABILITATION CITY OF LAKE CHARLES, LA SCOPE OF WORK The general scope of work for Project No. 2019-690 includes the replacement of the existing drainage line on the East side of Kirkman Street from Pine Street North to Pryce Street. This existing line is undersized and in poor condition. It will be replaced with new 18” diameter RCP and catch basins. The scope of work also includes the replacement of the sidewalk on the East side of Kirkman and the associated street pavement work.

City of Lake Charles, Louisiana Shattuck Street Bridge Lake Charles City Capital Project No. CP 3313 Project scope consists of lifting the ends of six 62 foot long Type III girders at the expansion joints (bents 7, 11, 12, 15, 18, and 21) to remove the existing bearing pads and replace them with new bearing pads. Each Type III girder will be lifted at one expansion joint at a time using ten (10) Enerpac RAC-502 (or equivalent) 50 ton hydraulic jacks.

City of Lake Charles, Louisiana Project No. 2019-687 Capital Project No. 3332 Port Wonder Utilities Extension The general scope of work includes the extension of a 12” diameter water main from the Northwest corner of the Old Harrah’s Parking Garage Tract West approximately 630 feet to the East property line of the City of Lake Charles Alligator Pond Tract, including fire hydrant assemblies and service connection points for the future Port Wonder Development, and the extension of a 10” diameter sewer main from the Northeast corner of the Old Harrah’s asphalt parking lot West approximately 1100 feet, including manholes. Pavement removed at the Port Wonder site will not be replaced. Pavement removed at the Old Harrah’s site will be replaced.

City of Lake Charles, Louisiana WWTP A BASIN A-03 REHABILITATION PROJECT CP3297 I. Scope of Services: A. The City of Lake Charles WWTP ‘A’ is currently under a Compliance Order (CO) with LDEQ. The approved CO schedule requires the rehabilitation of 13 sub basins in the WWTP ‘A’ Basin. The Sub Basins have been prioritized and ranked in order of severity of condition through wastewater modeling of the system. B. Based on review of CCTV data of the Sub-Basin, recommendations for repair have been made and a cost estimate has been developed. C. This scope of work covers the repair of the collection system in the WWTP A Sub Basin A-03. The Work includes, but is not necessarily limited to, the following: 1. Pipe bursting 8080 linear feet of existing 8” and 10” sewer to new 8” and 10”. 2. 30 point repairs to 8” sewer pipe at varying depths 0-12’. 3. 70’ of open cut replacement, 4 point repairs to 18” sewer pipe. 4. Replacement of approximately 132 active sewer service laterals. 5. Coating approximately 350 vertical feet of existing and new manholes. 6. Furnish and Install 3 new manholes.
