City of Lake Charles University Area Blue Sub-Basin Sewer Rehabilitation Capital Project Number CP3251 I. Background: The University area is one of the oldest areas in the City of Lake Charles. Wastewater flow studies have indicated excessive inflow and infiltration during rain events, causing peak flows up to 14 times the average daily wastewater rate. II. Scope of Services: Based on review of the CCTV data of the sewer line cleaning and CCTV project within the University area, the assessed pipelines have been designated as critically deficient and priority one deficient. Following review of the assessment the following scope of work has been developed. A. There is approximately 3,215 LF of 8” sewer pipe that has been identified as critical deficiencies and an additional 7,948 LF of 8” sewer pipe that has been identified as priority one deficiencies that will be rehabilitated as part of the base bid of this project. B. Manchac Consulting Group, Inc. will analyze the CCTV data provided, and rehabilitation methods will be refined upon review of the CCTV data during final design. Rehabilitation methods include pipe bursting, point repairs, cured-in-place lining, and manhole lining.

City of Lake Charles, Louisiana Lake Charles Civic Center Hurricane Laura Restoration – Exterior Repairs HL-14 1. Replace damaged and missing stucco wall panel system on South Wall. Restore to pre- disaster conditions. Color of finish on wall panels shall be selected by Owner. All existing similar panels on South wall will be patched and recoated with similar finish. Seal all joints. Approx 1300 SF 2. Replace existing temporary stud wall in area of missing wall panels at upper south wall and replace with new structural steel studs, insulation, and fireproofing. 3. Repair and replace 2’-3’ tall aggregate panel where missing at top of South Wall. Match existing if possible. Seal and paint to match existing aggregate fascia panel, approx. 100 LF. Provide structural steel stud framing. Seal all joints. 4. Repair damaged balcony walkway and railing at South elevation. Install new structural steel connection to support existing beams. (2 connectors) 5. Replace plaster soffit where removed at third level on the East side and where damaged under upper-level canopy at North wall. Replace plaster soffits at south end under failed walkway canopy where removed to repair structure. 6. Repair any damaged aggregate wall panels where holes occur. Approx. includes 6 locations 7. Miscellaneous repairs to aluminum storefront entrances. 8. Clean all surfaces and waterproof all exterior surfaces.

CITY OF LAKE CHARLES, LOUISIANA SOUTHEAST WATER TREATMENT PLANT NEW WATER TREATMENT FACILITY CAPITAL PROJECT NUMBER CP3262 I. Scope of Services: Contractor shall complete all Work as specified or indicated in the Contract Documents for Southeast Water Treatment Plant – New Water Treatment Facility project. The Work includes, but is not limited to, the following: • Construction of a new groundwater treatment facility predicated on the removal of iron and manganese which includes the following major components to facilitate the production, treatment, and distribution of potable water: o Four (4) water production wells. o Four (4) horizontal pressure filters. o Two (2) concrete, ground storage tanks. o Four (4) high service pumps. o Two (2) backwash pumps. o One (1) backwash recovery clarifier. o One (1) backwash recovery pump station. o One (1) sludge detention lagoon. o Chemical feed systems (chlorine, pot perm, fluoride, phosphate, and caustic). o Onsite backup generator with automatic transfer switch. o Buildings (Chemical Feed, Chlorine, and Filter Buildings). o Miscellaneous civil, mechanical, electrical, and SCADA components.

Project No. 2019-693 Capital Project No. 3308 Downtown Streetscape Phase II Bilbo St. between Division St. & Mill St. City of Lake Charles, LA The general scope of work for this project consists of reconstructing sidewalks at the intersections of Bilbo Street and Division Street and Bilbo Street & Mill Street in preparation of the new Woodring Apartment development in the 600 block of Bilbo Street. This will be accomplished by: • Reconstructing the sidewalks at the intersections to provide ADA compliant handicap sidewalk ramps • Removing the traffic signals at these intersections and converting them to 4-way stops • Constructing “bulb-outs” to further define existing streetside parking • Increasing the pavement width on the East side of Bilbo Street from Division Street to Mill Street to provide for additional parallel, streetside parking • Salvaging the existing bus-stop assembly and constructing a new bus-stop site on Mill Street This work will also require installation of new drainage features as well as the associated pavement work.

CITY OF LAKE CHARLES, LOUISIANA WASTEWATER TREATMENT PLANTS MISCELLANEOUS BUILDING REPAIRS CAPITAL PROJECT NO. HL-24, 25A, 29E, 29G, 29H, 32, 33, 81A, 81.D.1, 81M, 126E, 126C, 126D, 126J, 126K, 126F I. Background: Gusting winds, debris impacts, and heavy rain, associated with Hurricane Laura, incurred minimal to severe damages to multiple buildings located at Wastewater Treatment Plants A, B/C, and D. This project will serve to provide necessary repairs to reinstate building functionality to pre-storm conditions. Scope items have also been identified that would serve to upgrade these antiquated systems to the latest codes and standards. II. Scope of Services: Design and implementation of building repairs, in response to damages incurred as a result of Hurricane Laura (DR-4559-LA), to pre-storm conditions and regulations as imposed by governmental bodies with the appropriate jurisdictional authority.
