City of Lake Charles, Louisiana Southwest Water Treatment Plant Finished Water Storage and High Service Pump Station Expansion Capital Project Number CP 3245 I. Scope of Services: A. Expansion of the Southwest Water Treatment Plant, includes but is not limited to: 1. One (1) new 2.0 Million Gallon Type III, wire‐wrapped prestressed concrete tank. 2. One (1) new high service pump complete with motor starter and tie‐ins into the existing spares within the existing control panel. 3. One (1) new pump control valve and associated controls for the new high service pump. 4. One (1) new tank mixing system and associated air piping. 5. Interconnection site piping to and from new water storage tank. 6. A new header assembly to the high service pump station from the new and existing tank. 7. A complete recoating of the existing high service pumps, valves, header, etc from the existing ground storage tank to the termination of the discharge header within the pump and filter building. 8. Demolition of the existing facilities as indicated. 9. Miscellaneous electrical, instrumentation, civil, etc.

Project No. 2015-645 Capital Project No. 3160 6th Avenue Reconstruction Broad Street to 3rd Street City of Lake Charles, LA ~ The general scope of work for Project No. 2015-645, Sixth Avenue Reconstruction – Broad Street to 3rd Street, includes the following: (a) Reconstruction of roadways; (b) Storm drainage improvements; (c) Water Distribution System; (d) Sewer Line (e) Sidewalks

Capital Project No. CP3243 WWTP A BASIN A-12 REHABILITATION PROJECT City of Lake Charles, LA ~ A. The City of Lake Charles WWTP ‘A’ is currently under a Compliance Order (CO) with LDEQ. The approved CO schedule requires the rehabilitation of 13 sub basins in the WWTP ‘A’ Basin. The Sub Basins have been prioritized and ranked in order of severity of condition through wastewater modeling of the system. B. Based on review of CCTV data of the Sub-Basin, recommendations for repair have been made and a cost estimate has been developed. C. This scope of work covers the repair of the collection system in the WWTP A Sub Basin A-12. The Work includes, but is not necessarily limited to, the following: 1. Pipe bursting 10,930 linear feet of existing 6” and 8” sewer to new 8”. 2. Open cut replacement of 3,248 linear feet of existing 6” and 8” sewer to new 8”. 3. Replacement of approximately 265 active sewer service laterals. 4. Coating approximately 80 vertical feet of existing and new manholes.

Project No. 2016-653 24” Sewer Main Extension on LA-14 From McNeese Street North to McNeese Farm Road City of Lake Charles, LA ~ The general scope of work for Project No. 2016-653, includes the installation of a 24” sewer main extension by open cut and jack and bore, as well as precast concrete sewer manholes with interior liners.

Project No. CP3226 Lake Charles Civic Center Improvements - Part 3 City of Lake Charles, LA ~ A. The Work as defined in the General Conditions and described in the Contract Documents prepared by the Architect is summarized as follows: LAKE CHARLES CIVIC CENTER IMPROVEMENTS - PART 3 B. Renovations to existing second floor office space for new Meeting Rooms/Bridal Suites, and Conference Room. C. Renovations to existing Dressing Rooms at North end of Coliseum.
